It is covered by mucosa and runs from the alveolar ridge to the lining of the lip, tongue or cheek.

Frenula connect the mouth to other mobile structures, such as the lip or tongue.

Frenulum problems can occur in children and, if they are not treated, can persist into adulthood.

Types of oral frenulum and the problems they cause

Frenula don’t usually cause any trouble, but occasionally they can lead to prosthetic, speech, orthodontic or gum problems.

There are three types of frenulum:

Upper lip frenulum

This is a band of tissue running from the oral vestibule and the middle of the upper lip to the midline of the upper jaw.

This frenulum can cause a diastema, that is, a gap between the upper central incisors, especially in children who need early orthodontic treatment.

It can also make it harder to move the upper lip, which causes problems pronouncing certain words.

Long frenula with an abnormal attachment are usually treated with surgery and orthodontics; this removes the frenulum and closes the gap between the teeth.

Lower lip frenulum

Because this structure attaches to the interdental papilla between the lower incisors, it can cause gum problems that later require a connective tissue graft, due to the lack of keratinised gum in that area.

This increases the risk of gum inflammation and gum recession, which makes teeth look longer.

Short lingual frenulum

This frenulum connects the tongue to the floor of the mouth. If it is short, tongue movement is restricted.

This is probably the condition that worries parents most, as it is the most common in children and leads to reduced tongue mobility and problems with voice and speech.

To assess the degree of tongue-tie and whether treatment is needed, a very simple test is carried out to check whether the tip of the tongue can touch the palatal surface of the lower incisors with the mouth open.

We also check how far the tongue can be stuck out.

No treatment is needed if the tongue moves normally and speech develops without problems.

Even so, a minor procedure is sometimes needed to snip the frenulum slightly and allow the tongue to move naturally.

But… what is tongue-tie?

Tongue-tie (ankyloglossia), or a short lingual frenulum, is when this frenulum hasn’t developed sufficiently.

This frenulum connects the tongue to the floor of the mouth and is attached to the gum behind the lower teeth.

It shows up as varying degrees of reduced tongue movement, which can affect both speech and chewing.

Between the ages of 6 and 8, in cases of moderate tongue-tie, a conservative approach can be used, with mechanotherapy exercises to make the tongue move.

In other words, when tongue movement is only slightly restricted, seeing a speech therapist is enough.

When the restriction is greater, and usually once the child is past 6–8 years old, an oral surgery procedure called a frenectomy is carried out.

Types of frenectomy

A frenectomy is a very simple procedure that involves removing the frenulum completely; it takes about 30 minutes and is done under anaesthetic, so it doesn’t cause any discomfort. It can be carried out by simple excision or by rhomboid excision.

Frenectomy by simple excision

This simply involves cutting the frenulum with a scalpel or scissors and then stitching.

This type of surgery isn’t usually recommended because, despite how simple it is, it has more drawbacks than is desirable.

It doesn’t remove the excess tissue, the scar runs in the same direction as the frenulum, and the upper lip isn’t lengthened.

Frenectomy by rhomboid excision

This surgery is done in two stages.

First, the attachments of the frenulum to the vestibular gum and the lip are cut. Then the middle part of the frenulum and the transalveolar fibres are removed.

The lip is stretched to expose the frenulum and two haemostatic forceps are placed, one next to the gum and one next to the lip.

The attachments to the gum and lip are then cut.

Once the forceps are removed, a rhomboid-shaped wound remains, and the muscle attachments to be released are found in its deepest part.

Z-plasty

This is the technique used for upper lip frenulum surgery.

It is the most suitable option, as it removes the frenulum without leaving a scar in the same direction and also lengthens the upper lip.

The lip is lifted to put the frenulum under tension. Then a vertical incision is made along the middle of the upper axis.

Next, two parallel side incisions are made at a 60-degree angle to the first one, and the lower incision must be separated from the attached gum.

The incisions are then deepened and two flaps are raised and crossed over each other.

Finally, before stitching, any muscle attachments in the area must be removed. If you think you or your child may have a frenulum that is causing problems, book an appointment and we’ll assess it.

An oral frenulum is a band of fibrous connective tissue, muscle or both.
