What is Maxillary Orthopedics?
Traditional orthodontics aligns teeth, but orthopedics modifies the bones that support them. We use this specialty to treat skeletal discrepancies: when the upper jaw is very narrow, the lower jaw is set too far back, or there is a noticeable facial asymmetry.
Thanks to current technological advances, we can apply these treatments not only to children, but also to adolescents and adults, achieving facial harmony that improves aesthetics and, above all, respiratory and chewing function.

To move bones or perform seemingly impossible tooth movements, we need firm "anchoring points." At our clinic, we use state-of-the-art temporary anchorage devices that do the hard work discreetly and effectively.
Skeletal Disjunction (MARPE/MSE)
The solution to the narrow palate in adults. In the past, if an adult had a narrow palate, it could not be widened because the bone suture was already closed (fused).
What do we do? We use the MARPE (Miniscrew Assisted Rapid Palatal Expansion) technique. We place a special expander attached with micro-screws directly to the palatal bone.
The result: We were able to “open” the suture and widen the actual jawbone in adults. This not only corrects the crossbite but also greatly improves nasal breathing by widening the airways. No hospital surgery required!
Microscrews (TADs)
The invisible “anchor”. They are small titanium devices (like an earring) that we place on the gum or palate with local anesthesia in seconds.
What are they for? They serve as a fixed point of support for moving complex teeth. Imagine we want to move a molar backward without shifting the front teeth: we use the screw for support, not the other teeth. They allow us to close difficult gaps, straighten impacted molars, or correct a gummy smile.
Bone Distalators
Appliances that combine with microscrews to bring all the upper teeth back.
Advantage: They avoid having to extract healthy premolars in cases of severe crowding or when the upper teeth are very “protruding” (Class II).

In many borderline cases, the use of MARPE or skeletal anchorage allows us to camouflage and correct the bone defect, saving the patient from undergoing hospital surgery or a traumatic recovery.

We know that terms like "screws in the gum" or "bone expansion" can be intimidating. However, these anatomical areas have very little sensitivity. The placement of these devices is done with a small dose of local anesthetic (just like for a filling) and is a quick and surprisingly comfortable procedure.

Doubts about Orthopedics and Microscrews
These treatments are highly technical and innovative. It's normal to have questions about whether they're right for you or if they're uncomfortable. Here we answer the most frequently asked questions from our adult patients.
Does MARPE expansion treatment hurt?
The procedure is done under anesthesia and is painless. During the active phase (when you turn the screw at home), you may feel a strong pressure on your nose or cheekbones for a few minutes. This is a sign that the bone is remodeling. It is completely tolerable.
Will I see a gap between my central teeth?
Yes, and that’s good news. When expanding the palate with MARPE, a space (diastema) often appears between the central incisors. This is physical proof that the bone suture has successfully opened. This gap is then closed very quickly with orthodontics (braces or Invisalign).
How long do microscrews take?
It depends on the movement we need to make. They can be in the mouth from a few months until the end of treatment. They are very hygienic and, once you get used to them, you forget you’re wearing them.
Do I have an age limit for MARPE?
